RULES OF CONDUCT

Nurse Shawna 2015 and Shawna White expect clients to be respectful, responsible, and proactive in their health planning. Clients are expected to pay for the visit at the time of scheduling. If the visit is not paid for, but scheduled, the visit will not be acknowledged by Nurse Shawna 2015.

 

The client is responsible for implementing the discussed intervention(s) and keeping a journal for reference on how the intervention(s) worked or did not work and why. This helps with the review process of the care plan during the Health Follow-Up visits.

USER RESTRICTIONS

To create a safer and more respectful environment for all users, Nurse Shawna 2015 and Shawna White restrict bullying/harassment of any kind via online, in-person, live, or through communication pathways (such as, but not limited to, text, call, email, online meeting, etc.).

 

Nurse Shawna 2015 and Shawna White will take legal action as appropriate and exercise the right to cancel services at any time without refunding expenses paid to Nurse Shawna 2015.

 

Nurse Shawna 2015 and Shawna White’s Wix website is not HIPPA compliant, and it is recommended not to share personal health information (PHI) in comments, chats, meetings, or other ways that are not encrypted to protect and respect your health privacy. Nurse Shawna 2015 and Shawna White are not responsible for the client(s) or other parties related to the client(s) sharing PHI against HIPPA on Nurse Shawna 2015 or Shawna White’s websites, social media platforms, email, chat, meetings, calls, text, etc.

INTELLECTUAL PROPERTY RIGHTS

Nurse Shawna 2015 and Shawna White hold intellectual property rights for any (printed, emailed, virtual, etc.) blog posts, videos, papers, forms, notes, care plans, handouts, etc. that are created for the website, social media content, visit services provided, education services provided, or creative thinking and ideas involved with Nurse Shawna 2015 or Shawna White.

BUSINESS CONTRACT DETAILS

Nurse Shawna 2015 or Shawna White will not acknowledge a scheduled visit or Health Education booking that has not been paid for.

 

The client(s) are responsible for booking a visit or health education booking and paying for the service at the time of scheduling.

 

Nurse Shawna 2015 or Shawna White will provide health education, care plan(s), resources for further information, and support to the client(s) working on improving their health.

 

The client(s) will implement recommendations into their lifestyle and keep a journal of their experience from the recommendations for review purposes during visits:

  • The recommendation/intervention improved the outcome, and then explain how.

  • The recommendation/intervention is applied to the client(s) lifestyle, and then explain how.

  • The recommendation/intervention did not improve the outcome, and then explain why.

  • The recommendation/intervention is in the process of being used, and it is not clear if it is improving or not, but this is how the recommendation/intervention is affecting the outcome so far.

INTRODUCTION TO BUSINESS

Nurse Shawna 2015, or Shawna White, is meant to be a resource to help you take responsibility for your health care needs.

 

Nurse Shawna 2015 and Shawna White’s goal is for every person who wants to improve their health and understand their body to learn how. Everyone should know the basics of how their body works, how their chronic or acute diseases/problems affect the way their body works, how to manage their health, and how to communicate effectively with their provider to express what is going on and what their concerns are.

 

However, we are not born knowing all this information and it would be nice to have guidance from those who know about the body and health care. Education, resources, and self-management options are provided.

It is important to be established with a provider because Nurse Shawna 2015 and Shawna White do not replace your doctor’s visits, information, advice, orders, or opinions. You must keep your follow-up appointments, physicals, wellness visits, orders, etc. that your doctor has set with you.

 

Nurse Shawna 2015 and Shawna White do not place orders. Your doctor orders tests, procedures, medications, and other treatments, etc. Nurse Shawna 2015, or Shawna White, is here to help you navigate, effectively communicate, collect information needed, and have a say in your healthcare needs.

LIMITATION OF LIABILITY AND WARRANTY DISCLAIMER

Terms and Conditions of Nurse Shawna 2015 and Shawna White are subject to change.

 

Nurse Shawna 2015 and Shawna White are not liable/responsible for HIPPA information that the client shares with non-HIPPA approved sites. If the client wishes to share their personal health information (PHI), Nurse Shawna 2015 and Shawna White are not liable/responsible.

 

When emailing from Nurse Shawna 2015 or Shawna White Wix business emails, Google Mail (Gmail), Nurse Shawna 2015 or Shawna White will have confidential mode turned on. The client’s phone number will be needed to receive the SMS code to read the sent email. The email will have an expiration date and will not allow downloading, forwarding, copying, or printing of the email. Attachments can be viewed.

 

Please note that the confidential email could be sent to the clients' spam/junk folder.

 

To encrypt an email in Google Mail (Gmail), follow these steps:

  1. Open Gmail and click Compose to draft a new email.

  2. Click the lock icon that says Toggle confidential mode to turn confidential mode on.

  3. Review the expiration date and passcode settings before hitting the Save button.

  4. Once the settings are saved, you can Send the email.

 

If the client(s) need attachment(s) and/or form(s) they can download, then the confidential mode will need to be turned off to be sent via email. Attachments and forms can also be mailed to the client(s) if that is preferred.

REFUND POLICY

 

 

There are no refunds for any of the services provided, or not provided, to you by Nurse Shawna 2015 or Shawna White. Each visit takes days of research, planning, reviewing, and preparation to provide you with up-to-date information specific to your needs. Care plans work from a base care plan, but changes are made to make the care plan specific to your needs and goals. Interventions will be evidence-based practice (EBP).

CLIENT RESPONSIBILITIES

 

 

 

You are responsible for paying and scheduling your weekly visits, providing the needed information, applying what is discussed, working towards meeting care plan goals, taking an active role in being responsible for your health care, and making payment at the time of scheduling your visit.

INITIAL HEALTH ASSESSMENT

The initial health assessment visit is used to gather information, gain an understanding of your health goals, and discuss possible care plans. This visit can take 3 hours depending on the complexity of your health condition. Being prepared for this visit with the information needed prior to the visit helps to save time. There are forms to fill out to help you gather the needed information.

 

What information will be needed:

  • Medication list includes over-the-counter (OTC), prescription (Rx), herbal (ex., tincture, capsules, tablets, salves, creams, etc.), and any illegal drugs. I will need the dose, how often you are supposed to take the medication, how often you actually take the medication, the reason for taking the medication, and the prescribing provider's full name, work address, and work phone number.

  • Allergy list includes allergic reactions you have had (ex., to medications, foods/drinks, plants, ingredients, etc.), dates of the reactions, what the reactions were, what was done to resolve the reaction, what you did to prevent the reaction from occurring, and what the reaction was.

  • Medical history includes past and present health conditions (ex., diabetes, stroke, seasonal allergies, high blood pressure, sexually transmitted diseases, etc.) with dates of diagnosis and, if applicable, the dates of resolving the condition.

  • Surgical history includes past and present surgeries you have had (ex., Cosmetic, cesarean, removal of something, repair of something, etc.), any complications during or after surgery, the date of surgery, the facility the surgery was performed at with address and phone number, the surgeon's name, work address, and work phone number.

  • Social history can include (but is not limited to) questions related to education level, reading level, living conditions, family, friends, mental health, how you identify, sexual activity, use of illegal drugs, use of alcohol, use of marijuana, if you have an advanced directive or living will, occupations past and present, questions related to your community, transportation, fire safety, emergency planning preparedness, firearms present, etc.

 

During the visit:

  • I will review all the information provided on the forms with you to make sure I understand the information and have everything I need to move forward.

  • I will assess you during this visit. Assessments can include (but are dependent on location and are not limited to):

    • Vital signs (ex., blood pressure, heart rate, oxygen level, respiration rate, level of pain, height, weight, etc.)

    • Cognitive

    • Head-to-toe assessment (ex., skin, hair, nails, head, eyes, nose, mouth, ears, neck, chest, lungs, back, stomach, arms, legs, hands, feet, muscle, urine, bowel movements, head-to-toe, etc.).

      • Online head-to-toe assessment will be mainly subjective information with limited objective information.

      • In-person head-to-toe assessment will be both objective and subjective information.

    • Review of health journals (ex., blood pressure and heart rate log, blood sugar log, food log, responses to medications, responses to treatments and/or interventions, food/drink log, exercise log, weight log, etc.).

  • Review care plan ideas to determine our goals. We will work together to determine how to move forward with the care plan for each visit. Related education will be provided over the care plan at each visit, after the care plan is established.  

  • Answering current questions or concerns.

 

The initial health assessment visit is not meant to replace your doctor's visits, information, advice, orders, or opinions.

 

After the visit is complete, I take the information and study it to apply everything to you as a whole person and determine how to set up your care plans. You are responsible for scheduling your health follow-up visit at least 1 week from the initial assessment visit but no later than 2 weeks from the initial assessment visit (unless you contact Nurse Shawna 2015 or Shawna White to discuss setting an appointment later than 2 weeks after the initial assessment visit and receive a response stating the visit can be delayed by a set amount of time determined in the response).

HEALTH FOLLOW-UP

This follow-up visit is used after the initial health assessment visit, for all the following visits.

 

After the initial health assessment visit, you will need to schedule a health follow-up visit to allow us to discuss the care plans I put together and to clarify any questions I came across while going through your health information. You are responsible for scheduling your health follow-up visit at least 1 week from the initial assessment visit but no later than 2 weeks from the initial assessment visit (unless you contact Nurse Shawna 2015 or Shawna White to discuss setting an appointment later than 2 weeks after the initial assessment visit and receive a response stating the visit can be delayed by a set amount of time determined in the response).

 

During the visit:

  • I will review my findings from the prior visit (initial health assessment visit or last health follow-up visit).

  • We will review the care plan to determine if goals are progressing, met, or not progressing. We will work together to determine how to move forward with the care plan for each visit. Related education will be provided over the care plan at each visit.

  • I will assess you during this visit. Assessments can include (but are dependent on location and are not limited to):

    • Vital signs (ex., blood pressure, heart rate, oxygen level, respiration rate, level of pain, height, weight, etc.).

    • Cognitive (if needed).

    • Focused Assessment related to care plans or any concerns you may have.

      • Online focused assessment will be mainly subjective information with limited objective information.

      • In-person focused assessment will be both objective and subjective information.

    • Review of health journals (ex., blood pressure and heart rate log, blood sugar log, food log, responses to medications, responses to treatments and/or interventions, food/drink log, exercise log, weight log, etc.).

 

Heath follow-up visits are not meant to replace your doctor's visits, information, advice, orders, or opinions.

HEALTH EDUCATION

This is a Zoom meeting that allows me to teach about chronic diseases live.

  • My Zoom account will determine how many people I can have per class and how long the class can be. However, the price will be the same with any time changes.

  • Questions can be answered after the Health Education presentation with the remaining time.

    • It is best to have questions written down prior to the Zoom meeting and and write down additional questions during the presentation.

    • It is best to have paper and something to write with to take notes from the presentation.

    • Resources will be provided from the presentation (ex., resources from where information was gathered, related sites, related logs, etc.)

  • Any further information outside of the Zoom health education meeting will require signing up for services.

    • Initial Health Assessment Visit if you are new to or have completed services with Nurse Shawna 2015 or Shawna White.

    • Health Follow-Up Visit if you are currently using Nurse Shawna 2015 or Shawna White.

 

Zoom health education meetings are not meant to replace your doctor's visits, information, advice, orders, or opinions.
